Michigan-specific coverage provisions
Michigan insurance law includes the following provisions that affect individual health insurance:
- The reference to 24 months in the definition of a preexisting condition is changed to 6 months.
- Provider Network Continuity of Treatment: If your provider leaves the network while you are receiving treatment for an "injury or illness", your first subsequent visit win be covered as if your provider were still in the network, and we will notify you that the provider is no longer a network provider so that you may choose a new network provider.
- Grievance Procedure Information Phone Number: (8OO) 657-8205. Upon request, we will provide you with the telephone number for the Michigan Department of Consumer and Industry Services.
- Expenses incurred for diagnosis and treatment of pain are covered expenses the same as any other illness or injury.
